The Causes of Hair Loss: More Complex Than You Think
Hair loss is not a single condition — it is a symptom with dozens of potential underlying causes. Androgenetic alopecia (genetic pattern hair loss) is the most common cause in both sexes, affecting an estimated 50 million men and 30 million women in the United States alone. But the list of other causes is long: telogen effluvium (diffuse shedding triggered by stress, illness, nutritional deficiency or hormonal change), alopecia areata (autoimmune hair loss), traction alopecia (from tight hairstyles), scalp conditions including seborrhoeic dermatitis and psoriasis, thyroid dysfunction, iron deficiency, and the side effects of various medications.
Accurate diagnosis requires a structured assessment that goes beyond looking at the scalp. Trichoscopy — dermoscopic examination of the scalp at high magnification — allows detailed evaluation of follicular density, miniaturisation patterns, scalp inflammation and other microscopic features that guide diagnosis. The Hair Growth Cycle
Understanding why hair falls out requires understanding the normal hair growth cycle. Each follicle cycles independently through three phases: anagen (active growth, lasting two to seven years), catagen (transition, two to three weeks) and telogen (resting and shedding, two to four months). At any given time, approximately 85 to 90 percent of follicles are in anagen and 10 to 15 percent in telogen.
Telogen effluvium occurs when a significant proportion of follicles are simultaneously pushed into the telogen phase by a triggering event — major surgery, severe illness, childbirth, rapid weight loss or extreme stress. The resulting diffuse shedding typically begins two to four months after the trigger and resolves over six to twelve months once the underlying cause is addressed.
Medical Treatments for Hair Loss
Two medications have strong evidence bases for androgenetic alopecia: minoxidil and finasteride. Minoxidil is a topical vasodilator applied directly to the scalp that prolongs the anagen phase and increases follicle size. It is available over the counter and effective in both men and women. Finasteride is an oral medication for men that inhibits the conversion of testosterone to DHT — the hormone responsible for follicle miniaturisation in genetic hair loss. Both treatments require ongoing use to maintain results.
Dutasteride is a more potent alternative to finasteride, and spironolactone or oral minoxidil may be used in women. Low-level laser therapy (LLLT) has emerging evidence supporting its use as an adjunct to other treatments.
When Surgery Is the Right Answer
Hair transplant surgery is appropriate when there is established, stable pattern hair loss with adequate donor supply and a realistic expectation of natural-looking coverage. Surgery permanently relocates follicles from the permanent donor zone to the balding recipient area — the transplanted follicles retain their genetic resistance to DHT and grow for life.
Surgery is not appropriate as a first-line response to active, unstable or diffuse hair loss — medical treatment and diagnosis should precede any surgical decision.
